Small Entity?

As I was reading through the forms last night, I came across a few documents which involved the small entity application for the PPA fee . I am a small entity, I know this, but do I need to visit a person to perform an oath??

OR do I just get to check the little box which says small entity.

Looks like your getting ready to file your own PPA. Take your time and don't rush through it.
